Registered Nurse- MDS Coordinator-F/T

We currently have exceptional opportunities for RNs, LPNs, & CNAs:


A current New York State licensed Registered Professional Nurse who possesses a Registered Professional Nursing degree from an accredited college school of nursing program. Bachelor’s degree preferred. Two years of clinical nursing experience and one year geriatric experience strongly preferred. Demonstrated leadership ability in nursing practice in a Long Term Care setting required. Must possess and maintain all necessary certifications for CPR/BLS and other certifications, in-services as required by this facility. Completion of the MDS 3.0 training experience required. Employee must possess a working knowledge of the Medicare/OBRA assessment and regulations in a Long Term Care Facility. Employee must possess working knowledge of EMR programs, i.e. ADL, PCC and general computer and associated skills.

Registered Nurses (with Long Term Care exp)

  • RN-Unit Manager- F/T Days–(Flexible to include 1-2 weekend days every 4-6weeks)
  • RN Supervisors – F/T Eves, P/T Nights
  • RN Staff Subacute- F/T, P/T Eves & Night Shifts


  • LPNs – Days & Nights (Full Time & Part Time)


  • All Shifts (Full Time & Part Time)

All candidates must have appropriate NYS Licensure/Registration/Certification.

Must be able to bend, turn, push, pull and lift up to 50 lbs without restrictions or limitations. Prolonged, extensive or considerable standing/walking, lifting, positioning, pushing, transferring patients, considerable reaching, stooping and bending.

Food and Nutrition

Nutritional Aide - Per Diem


Varies, to include rotating weekends & holidays

General objectives

To engage in service and sanitation activities related to food service for resident, patient, staff, volunteers and special functions in accordance with established norms for job performance, productivity, safety and sanitation

Serving cafeteria guests, Assist Resident Tray Line, Sanitation of kitchen areas, disposal of garbage and receptacles.

The candidate must possess 2+years’ experience, preferably in a healthcare setting.

As required by the Americans with Disabilities Act (ADA). The Candidate will be required to:

  • to be able to walk about
  • to be able to climb stairs
  • to be able to carry approximately 5-10 lbs. (documents)
  • must possess visual and auditory acuity with or without assistive/corrective devices
  • must exhibit the ability to communicate (transmit and receive) verbal and written information in an effective time efficient manner
  • must exhibit the physical ability to provide the full scope of required job tasks. This includes but is not limited to lifting, prolonged sitting and repetitive movements
  • must demonstrate the ability to comprehend verbal and written instructions and follow through with logical plan and course of action must demonstrate the ability to perform assigned tasks independent without the need for constant re-direction.
